LEVELS OF CARE

Long- Term Care

Long-term care is provided for residents who can no longer live independently. A plan of care is developed for each resident which addresses the nursing, medical, social services, nutrition, activities and rehabilitation needs of the resident. Families are encouraged to be a part of the care plan team.

Rehabilitative Therapy

Our goal is to help individuals regain lost function or develop compensatory strategies, which may allow them to return to their homes and their communities. We provide a team approach to care, utilizing the services of specialty physicians, nurses and therapists to address disabilities and functional limitations.

Rehabilitation at The Legacy Rehab & Care Center is designed to:

Our rehabilitation team creates an individual therapy program for each resident developed to help the patient take advantage of latest rehabilitative techniques and strategies.
